Please ensure that you submit your Travel Questionnaire at least six weeks before you are due to travel.
If you require any vaccinations relating to foreign travel you will need to complete the online travel questionnaire first – Travel Questionnaire.
Please complete and submit this to the Practice. Following receipt of your completed travel questionnaire our Practice Nurse Team will review the Questionnaire within 5 working days. Once the Questionnaire has been reviewed you will be contacted to discuss your travel arrangements and to advise you of any appointment required to receive the vaccine/s.
It is important to complete and submit your travel questionnaire as early as possible – at least 6 weeks before you travel. These vaccines have to be ordered as they are not a stock vaccine. Please be aware that you need 2 weeks before you travel to allow the vaccines to work. Travel vaccinations are routine (non-urgent) appointments and you need to ensure that you allow time for the travel questionnaire to be reviewed and sufficient time for a routine appointment to be available.
Not all travel vaccinations are included in the services provided by the NHS. If you require a non-NHS travel vaccine you will need to contact a Travel Clinic to arrange this. We only provide NHS travel vaccinations at this Practice.
The Vaccines listed below are the NHS Travel Vaccinations which we can provide (subject to requirements) at the Practice following receipt and review of your questionnaire by our Practice Nurse
- Hepatitis A
- Combined hepatitis A and B
- Typhoid
- Combined hepatitis A and typhoid
- Tetanus, diphtheria and polio
- Cholera
If you require any of the vaccines listed below, these are not covered by NHS travel vaccinations and you will need to arrange to attend a Travel Clinic to receive these vaccines and pay accordingly.
- Japanese Encephalitis x 3 doses
- Tick Encephalitis x 3 doses
- Rabies x 3 doses
- Hepatitis B x 3 doses including blood test
- Meningitis AWC x 1 dose
